Chronic Disease Manager
Job Summary
Under direction of the Associate Executive Director this individual
will be responsible for the general operation of the diabetes clinic and community
health prevention programs and serve as a liaison between the county clinic
and El Concilio. The individual hired will supervise and direct clinic and
community staff in the performance of their duties, monitor progress and performance,
data entry, record keeping and maintenance of program documents. In addition,
assist in program development, planning and program reporting. Familiarity
and experience with databases essential.
Typical Tasks:
- •
Manage the general operations of Nuestro Canto de Salud Diabetes Clinic;
• Maintain and develop new chronic disease programs for secondary prevention
services;
• Perform quality assurance measures for efficiency of data entry;
• Understand File Maker Pro data system to be able to troubleshoot, monitor
data and problem solve;
• Update all health education presentations used by ECSMC programs as needed;
• Establish a centralized policy & functional manual for coordinated
and integrated services;
• Provide training and technical assistance to ethnic-based agencies/groups
interested in promoting chronic disease prevention;
• Represent El Concilio in community collaboration and organization events;
• Performs other related duties as assigned by Supervisor.
Minimum
Qualifications:
- •
4 years College education in health or social sciences;
• 3 years experience which demonstrates the possession and application of
the knowledge and abilities;
• Valid and Current California License and Automobile Insurance (Proof of
Insurance required).
• Standard community resources of health education; management and supervision.
• Program planning process and Consultation techniques;
• Respect for multi-cultural and spiritual diversity;
• Understanding of community interdependence;
• General knowledge of preventive medicine and epidemiology.
• Knowledge and experience working with computers and data bases;
• Planning, implementation, and evaluation of community health education
programs;
• Provide health education consultation and develop relationships with a
wide range of individuals, organizations and entities;
• Communicate effectively, both orally and in writing in both Spanish and
English, with individuals and groups from varying cultural, ethnic and education
backgrounds;
• Follow organizational policies and procedures, government laws, regulations,
and guidelines.
